Education & Training: Resource materials are available to caregivers and their families through a lending library. These materials include books, video tapes and cassettes. Caregiving training courses are offered at intervals throughout the year. Care Management: Caregivers receive an initial in-home assessment form a social worker who makes supportive services referrals. Based on client's needs, the social worker may provide monthly monitoring to ensure care plan goals are met. Respite: To enable caregivers to be temporarily relieved of caregiving responsibilities. Respite services can supply a trained person to provide support at home. These services are limited and temporary; however, respite is very important for a caregiver's long-term health and well being. Supplemental Services: Home delivered meals, assistive devices homemaker and transportation service are available on a limited basis to the care receiver, as well as the caregiver.
