Comprehensive 5-session series on Diabetes self-management skills according to American Diabetes Association curriculum. Taught by nationally certified Diabetes educators, Registered Nurses, and Registered Dietitians. Education provided to groups: provided on a one-to-one basis to patients with visual, auditory, cognitive, or developmental challenges. Insulin pump therapy provided by Nurse Practitioner. Continuous glucose monitoring for 7 days provided by Registered Nurse and Nurse Practitioner. Accepts SSI and Medi-Cal.
