It is a special home visitation program for first time moms and their babies. The mother and child are visited in their home regularly over a period of 2 years by public health nurses. First time mothers that are income eligible and living in the 91752, 92501, 92503, 92505, 92507, and 92509 zip codes. You are eligible for the program in your 6 months of pregnancy. The earlier you apply the better. A Public Health Nurse comes into your home, and provides individualized care to meet your needs before and after delivery. This includes monitoring pregnancy for complications, information on what to expect at the time of delivery, and how to care for yourself and your child at home.
